Rosacea is a common but poorly understand skin condition. About 16 million people have it and most don't know. The acne-like disease can affect the face, chest and back. Although there is no cure for rosacea, the key to keeping it under control is to see your doctor as soon as possible to begin treatments, and to continue treatment for as long as your dermatologist recommends.

Overview

Rosacea is a chronic skin condition that usually begins on the cheeks, chin, nose or central forehead and may gradually spread over time to include the ears and upper torso. It most commonly affects people over the age of 30, first appearing as a persistent flush or blush on the face. Over time, tiny blood vessels may become visible as well as pimple-like bumps which may at first be mistaken for acne. Left untreated, these symptoms can worsen. The nose can become fleshy and swollen in some people. Others may have eyes that feel irritated and bloodshot.

Those who are at greatest risk include men and women who:

  • Are fair-skinned
  • Are of Scandinavian or Celtic heritage
  • Have a family member with rosacea
  • Have a family member with bad acne
  • Have had bad acne themselves

Signs and Symptoms

Four subtypes of the long-term disease have been identified, each of which is associated with different signs and symptoms:

Subtype 1 - Facial redness (Erythematotelangiectatic rosacea)

  • Redness, flushing and blushing in the central areas of the face
  • Skin appears swollen
  • Spider veins (broken blood vessels) are visible
  • Stinging, burning and increased sensitivity in the affected area
  • Skin that is dry, rough or scaly

Subtype 2 - Acne-like (Papulopustular rosacea)

  • Breakouts similar to acne accompanied by redness
  • Very oily skin
  • Stinging, burning and increased sensitivity in the affected area
  • Spider veins (broken blood vessels) are visible
  • Presence of plaques - raised skin patches in the affected area

Subtype 3 - Thickening skin (Phymatous rosacea)

  • Bumpy skin texture
  • Thickening of the skin, most commonly on the nose
  • Swollen cheeks
  • Bumpy texture to the skin
  • Spider veins (broken blood vessels) are visible
  • Enlarged pores

Subtype 4 - Eye involvement (Ocular rosacea)

  • Eyes feel gritty, irritated; burning and stinging sensations may occur
  • Eyes appear watery or bloodshot
  • Eyes may be very sensitive to light
  • Vision may be blurry
  • Broken blood vessels may be visible on the eyelid

Diagnosis and Treatment

This skin condition is diagnosed from a thorough exam to look for visible signs, as well as a comprehensive medical history. Once the subtype or subtypes are determined, your dermatologist will be able to determine the best course of action. Treatment may include topical medicines, emollients, topical and oral antibiotics, light treatment including laser procedures and removal of thickened skin. For ocular rosacea, you may be instructed on how to wash your eyelids and be prescribed medicated eye drops.

Outcomes and Follow-up

Currently, there is no cure for rosacea, and many patients have flare-ups of the condition for years after the initial occurrence of the skin disease.
